Before You Begin
Decide what information the calendar form should collect before opening the form builder.

Step 1: Create a Custom Form
Build and save the form that will collect information during the calendar booking process.
Open the Form Builder
From the BizStackPro dashboard, go to Sites → Forms, then open the Builder.
Add a New Form
Click Add Form. Start with a blank form or select an available form template.
Name the Form
Enter a clear internal name that identifies the purpose of the form, such as Consultation Intake.
Add the Core Contact Fields
Add the primary contact information needed for the booking. Common fields include:
- First Name
- Last Name
- Phone
Add Location Fields When Needed
Local or service-based businesses may also need to collect location information such as:
- Address
- City
- ZIP Code
Add Questions for Your Workflow
Add custom questions that help you understand the contact's needs before the appointment. Examples include:

From the BizStackPro dashboard, go to Calendars → Settings.
Select the Calendar

In the calendar settings, open the Forms & Payments tab.
Choose the Custom Form
Select the form you created from the available dropdown list.
Choose the Display Order
When the option is available, choose whether the contact should complete the form before selecting an appointment time or select the appointment before completing the form.

Add a post-booking message or redirect URL when one is needed for your appointment workflow.
Save the Calendar
Review the selected form and calendar options, then click Save.

Open the Public Booking Link
From Calendar Settings, click the applicable share or arrow icon to open the public booking link.

Submit a Test Booking
