Signature Custom Field in Forms and Surveys

What the Signature Field Does

The Signature custom field lets a user sign directly inside a form or survey. When the form is submitted, BizStackPro stores the signature in the contact record and adds verification details so you can validate authenticity.

Step 1: Create the Signature Custom Field

1. Go to Settings → Custom Fields.
2. Click Add Field (or + Add Field).
3. Select Signature and click Next.
4. Choose a folder/group where it should be stored (recommended).
5. Click Save.

Step 2: Add the Signature Field to a Form or Survey

1. Open Sites → Forms (or Surveys) and go to Builder.
2. Create a new item or click Edit on an existing form/survey.
3. Locate Custom Fields in the left panel.
4. Select your Signature field and drag it into your layout.
5. Optionally rename the label (example: “Sign Here”).
6. Click Save, then Preview to test the signing experience.

Authenticity, Validation, and Exports

• Each signature submission generates a timestamp and an encrypted hash.
• Those verification details are stored with the submission for traceability.
• When you export submissions to PDF, the signature + verification details can be included for records.

Step 3: Find Stored Signatures in Contact Records

1. Go to Contacts and open the contact record.
2. Look under the custom field folder/group you assigned (example: Signatures).
3. Open the signature entry to view the stored value (and any related details).

If you collect multiple signatures per contact (contracts, waivers, approvals), using a dedicated folder makes it much easier to track.
